X-Ray/MRI Review

To help determine whether an office appointment may benefit you, the physicians at the American Hip Institute will review your hip, knee, or shoulder imaging, including X-rays, MRIs, or CT scans.
Please send all required materials to the address provided below and carefully follow the instructions to ensure your records are complete. Incomplete submissions may delay your review.
If you would like your imaging returned, please include a self-addressed, stamped envelope with your package.

- A completed Intake Form (download here)
- A signed Consent for Review of Records & Imaging Studies Form (download here)
- A copy of your insurance card
- Diagnostic imaging and reports: X-rays, MRI/Arthrograms, and CT scans, along with the reports that correspond to the imaging. Images should be on a disc.
- Office notes from previous/current treating physicians
- Operative reports and images specific to the affected joint.

A Care Specialist will contact you via phone when we have received your records to confirm receipt, complete your registration, and route your materials to the clinical team for review. They will ask you for some additional information needed to enter you into our medical record system.
A member of the American Hip Institute clinical team will review your records with our physicians and call you directly via phone to discuss whether an office appointment would be beneficial for you. Please note that there is no charge for medical record reviews outside of Illinois, Wisconsin, and Indiana. Carefully review the disclaimer below to clearly understand the scope of this review.
